COVID-19 Jind Lockdown Exemption Card form

Application for exemption card during lockout in Jind due to outbreak of COVID-19. Please fill in your correct details.
The applicant will apply through the given link and after providing all the mandatory information and successful submition, applicant will get a Reference No.
All the applied applications will be visible in authority user i.e. DC user. Once the application gets approved from DC user the applicant will be able to download the pass i.e E-Pass from the link given on district website after providing Reference No. which he/she got during the apply of the application.
E-Pass comes with QR code hence the authenticity of the Pass can be checked if required.
